In this course Michelle is joined by the one and only Kim Conant, LPN, Special Needs Care Coordinator with Palmetto Pediatrics. Interprofessional practice approach… done right!!... is the theme of this episode. Kim explains her unique role at a pediatrician’s office, how she works collaboratively with a physician to advocate for referrals to Speech Therapy for her patients, and explains barriers to accessing to ST. Want to know more about how we as a profession can improve continuity of care with our patient’s medical team, well then tune in and let Kim shine her light and educate!

Objective

By the end of this PodCourse, participants will be able to:

  1. Define the role of a Special Needs Care Coordinator and describe 2 benefits they offer to a patient and their pediatrician.
  2. Identify and explain 3 considerations that a physician has to make prior to making the referral for speech therapy, and how a Special Needs Care Coordinator can advocate for that referral.
  3. Identify and describe 3 examples of how we as Speech-Language Pathologists can improve the continuity of care for our patients by engaging with the rest of their medical team. 

About Kim: Kim Conant, LPN been a pediatric nurse for 42 years, and 26 of those years she has spent with Palmetto Pediatrics.  She started the unique, (at the time), Special Needs Care Coordination/Medical Home program in September of 2002.   Since that time, 16 years ago, she has trained numerous other Care Coordinators. Over her career, Kim has had the opportunity to work with countless multi-disciplinary teams to support families of children with special healthcare needs. Kim feels that an interprofesional practice approach is critical for each special needs child, and their families, to help them reach their ideal outcomes.  In her "spare" time, Kim enjoys gardening, reading, scrap-booking, traveling, and spending as mush time as possible with her 3 grandchildren.

Course Timeline:

0-10: Introductions and Backgrounds

10-20: Discuss the role of a Special Needs Care Coordinator and benefits they bring when treating a pediatric patient.

20-30: Discuss Physician considerations when making a referral for Speech Therapy and how a Special Needs Care Coordinator can assist.

30-45: Discuss how a Speech Language Pathologist can engage in IPP by improving continuity of care with the Special Needs Care Coordinator.
